- Board Certified / Eligible Urologist
- Active Indiana License or in process
- General Urology with laparoscopic surgery
- Call 1 : 3
- Support a primary care referral base of 39 employed primary care physicians / advanced practitioners
- Excellent working relationship between Administration, Board of Directors and Medical Staff with half of Board seats held by physicians
- Competitive salary and work RVU incentive paid quarterly
