Registered Physical Therapist (PT) – Home Health

Direct Hire | Clermont County | Flexible Schedule | $75 / Visit

What You’ll Do :

Provide physician-prescribed physical therapy in patients’ homes

Conduct initial assessments, develop care plans, and reassess every 30 days

Perform skilled therapy procedures and teach patients and families home exercises and activities of daily living

Collaborate with physicians, families, and other home health personnel to optimize outcomes

Document visits, OASIS assessments, progress notes, and discharge summaries accurately and timely

Participate in in-service programs, patient care conferences, and Quality Assessment & Performance Improvement activities

Supervise Physical Therapy Assistants and Home Health Aides as assigned

Qualifications :

Graduate of an accredited Physical Therapy program approved by CAPTE, APTA, or relevant governing bodies

Licensed in Ohio (or applicable practicing state)

Minimum of 2 years’ experience preferred

Strong initiative, independent judgment, and commitment to patient-centered care

Alignment with the philosophy and goals of our agency

Working Environment :

Travel to patient homes and work in-office as needed

Physical activity including bending, lifting, and standing for extended periods may be required
