Job Description

Job Title : Registered Nurse

Experience : Minimum of 1 full year of adult home health experience or 1 full year of hospice experience, depending on the specialty

License : Must have a valid nursing license in hand

Certifications : CPR, BLS

Must-Have :

Proficiency in HomeCare HomeBase (HCHB) EMR and OASIS documentation
Reliable transportation, current driver's license, and liability insurance
Willingness to travel up to 75 miles from the home clinic

Job Description :

Coordinate and direct the provision of home nursing care according to physicians' orders based on agency policies and procedures.

Directly supervise and evaluate home health aides and licensed practical / vocational nurses.

Conduct home visits and attend periodic office meetings.

Handle types of cases such as Start of Care, Wound Care, and Follow-up Visits.

Participate in weekend and on-call rotations as required.

Average patient load involves visiting 5-10 patients per day, translating to 6-8 productivity points.

Ensure all documentation is completed within 24 hours of the appointment with the patient.

Meet weekly productivity points of 30, equating to 40 hours per week.

Be open to floating to another nearby clinic to pick up patients if necessary.
