We are seeking a Case Manager RN to join our team! You will be responsible for the assessment, diagnosis, and treatment of assigned patients.

Position Name :   Case Management

Available Shifts :  Evening shift : 12 : 00 PM – 8 : 30 PM – we can accommodate 11 : 00 AM – 7 : 30 PM if need be – Monday – Friday / No Weekends

Salary Range :  Min - $32.91 / hr - $41.14 / hr Midpoint (10 years of experience) - $45.25 / hr Max (10+ years of experience)

Shift Differentials :  Evenings - $2.00 ; Weekends - $2.00

Sign on :  up to $15,000

Relocation assistance :   Case by Case

Side Note

• The rotating weekend requirement is only 2 weekend days per month. It could be one day on two separate weekends
