JOB SUMMARY

The Ambulatory Care Manager works in coordination with the leadership team(s) in the inpatient and ambulatory care realm in an assigned setting to ensure transition of patients is appropriate and seamless. This position provides support for the patient, their family / healthcare partner and bridges the gap between the inpatient and ambulatory care setting.

DUTIES AND ESSENTIAL FUNCTIONS

Provides assigned patients support and assists in transitions between the inpatient and ambulatory settings and immediately resolves issues to enhance their health.

• Overall performance of below indicated essential job duties :

Assist patients and their family / healthcare partner in the inner workings of the health system to maximize their healthcare resources.

Completes a patient-specific plan of care that includes “touch-base” points.

Provides primary care physicians feedback on patient-specific information / needs as appropriate.

Knowledgeable of and appropriate application of the nursing process, professional theory, practice and procedure and assess nursing needs of acute and chronically ill patients and their families

Effectively teaches patients and families in accordance with the multi-disciplinary plan of care

Exhibits enhanced knowledge regarding payer plan, resources available in said plans, and communicates cohesively in lay terms to the patient related to these plans.

EDUCATION / LICENSE / CERTIFICATION / OTHER REQUIREMENTS

Minimum requirement :

Graduate from accredited program for Registered Nurses.

Maintains active licensure in the State of Kentucky as a Registered Nurse or other NLC state with multistate privileges.

Preferred qualifications :

Bachelor’s Degree in Nursing

WORKING ENVIRONMENT

Constantly works near moving mechanical parts

Constantly exposed to blood and / or body fluids and infection.

Occasionally exposed to toxic or caustic chemicals

Occasionally exposed to risk of radiation

The noise level in the work environment is usually moderate.

PHYSICAL DEMANDS

Occasionally required to maintain in a stationary position (standing or sitting).

Frequently required to walk and move about.

Frequently required to operate a computer and other equipment.

Frequently required to use fine motor skills to handle and operate instruments / devices.

Constantly required to lift and / or move items up to 10 pounds.

Frequently required to lift and / or move items up to 50 pounds.

Occasionally required to lift and / or move items up to 100 pounds.

Constantly required to communicate with patients and other care team requiring talking and hearing.

Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, and depth perception.

Occasionally required to position self in order to meet patient needs.
