Description :
The physical therapist provides case management and direct patient care within a team environment.

The qualified therapist assists in evaluating the level of function, helps develop and revise the plan of care, prepares clinical and progress notes, consults with family and other personnel, and participates in in-service programs.
Responsibilities include ensuring care delivery services align with the organization's vision and meet customer needs.
The therapist promotes effective working relationships and adheres to the dress code policy to enhance the organization's image.
They utilize resources, supplies, and time efficiently to contain costs and improve service value.
Positive, respectful communication with external sources, patients, families, and staff is essential.

Direct patient care duties involve developing and revising treatment and discharge plans in conjunction with the patient, family, and care team.
The therapist provides supervision of support staff to ensure patient health, safety, and compliance with the plan of care.

Implementation of home therapy programs, including gait training and transfers, is part of their responsibilities.
The therapist periodically evaluates patient progress and makes necessary adjustments to the therapy program.
They guide para-professionals in body mechanics and the use of equipment.
Quality supervision of patient care services, including home health aides, is overseen by the therapist.
The physical therapist engages positively with patients and other care providers to maintain effective relationships.
