Responsibilities :

  • Deliver clinical guidance and support to patients while providing therapeutic recommendations to physicians, nurses, and other hospice staff.
  • Review and interpret medication histories / profiles, assess patient adherence, and recommend adjustments to ensure safe and effective medication use.
  • Apply clinical pharmacy knowledge, considering pharmacokinetics and pharmacodynamics, to optimize drug therapy and achieve positive health outcomes.
  • Address patient and provider inquiries promptly, offering clear and professional resolutions.
  • Accurately process, verify, and dispense prescriptions in compliance with regulatory standards.
  • Ensure medications are stored securely and under proper conditions at all times.
  • Help maintain an organized, efficient, and compliant pharmacy environment.

Qualifications :

  • Strong written and verbal communication skills, with the ability to engage effectively over the phone and in person while maintaining a patient-focused approach.
  • Proven problem-solving and prioritization skills in a fast-paced setting.
  • Capable of working independently while also contributing to a collaborative team environment.
  • Ability to remain professional and composed under pressure.
  • Active and unrestricted pharmacist license in good standing with the State Board.
  • Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy.
