This important position will lead and oversee the community’s Health & Wellness program to ensure high-quality resident care in accordance with applicable state regulations and community policies. Responsibilities include clinical leadership, resident assessments and service plans, medication management oversight, team training and supervision, quality assurance, and effective communication with residents, families, providers, and internal partners.
Essential Duties & Responsibilities
- Clinical leadership & compliance
Plan, organize, and direct daily Health & Wellness operations in alignment with state AL / MC regulations and community policies.
Maintain survey / readiness standards; complete audits, corrective actions, and quality improvement initiatives.Ensure required health screenings (e.g., TB) and offered vaccinations (e.g., Hepatitis B per policy) are coordinated and documented.Admissions & level of careComplete pre-admission assessments to determine fit within the licensed scope of care; coordinate move-ins, transfers, and move-outs based on clinical criteria and regulatory requirements.
Reassess residents per regulation and when change of condition occurs; update service plans accordingly.Resident care coordinationManage resident health needs; coordinate with physicians, pharmacies, home health, therapy, and other providers.
Communicate timely, professionally, and appropriately with residents and families regarding health status and care plans.Medication managementOversee medication administration systems (MAR / eMAR), storage, counts, and documentation; ensure accurate reconciliation and accountability.
Report and address medication variances per policy; train and validate competency for medication-delegated staff consistent with state scope.Team supervision & developmentSupervise and schedule nursing / wellness team members and other staff performing delegated health tasks.
Provide onboarding, ongoing education, skills validation, and coaching; promote appropriate delegation and teamwork.Lead daily stand-ups / huddles; ensure issues from the electronic record are addressed.Incidents & change in conditionReview all incident / accident reports; complete follow-up, documentation, care plan updates, and notifications.
Escalate significant changes in condition (e.g., new aggression / combative behaviors in dementia, frequent falls, acute decline) to the Executive Director and regional / clinical leadership per policy.Direct care within scopeAs needed, provide hands-on support (e.g., treatments, medication administration) within licensure, policy, and state regulations to maintain resident safety and care continuity.
Documentation & systemsEnsure timely, accurate documentation in the electronic health record (EHR).
Maintain required logs, audits, and reports (e.g., infection control, falls, weights, vitals, service plan reviews).OtherParticipate in on-call rotation and provide after-hours guidance as needed.
Perform other related duties as assigned.Qualifications
Active RN or LPN license in the state of employment (RN preferred where required by regulation).3+ years of senior living, assisted living, memory care, or related clinical experience; leadership / supervisory experience preferred.Strong knowledge of dementia care practices and behavior management.Current CPR certification (or ability to obtain promptly).Demonstrated communication skills (written and verbal) with residents, families, staff, and providers.Excellent organization, attention to detail, judgment, problem-solving, and critical-thinking skills.Proficiency with EHR / eMAR and basic computer applications.Ability to work under pressure, manage multiple priorities, and maintain a compassionate, resident-first approach.Availability for a flexible schedule, including evenings / weekends for resident needs, events, and on-call.Benefits & Perks
