Physical Therapist

We're seeking a dedicated and skilled Physical Therapist to join our team. In this role, you'll be instrumental in helping patients regain mobility and improve their quality of life through comprehensive therapy programs. You'll work collaboratively with a multidisciplinary team, providing expert care and guiding patients toward greater independence.

Essential Duties and Responsibilities

  • Patient Evaluation & Treatment :
  • Evaluate patients within 24 hours (or 1 business day) of physician referral, documenting any delays.
  • Develop and implement effective, physician-approved treatment plans.
  • Provide direct patient care according to established treatment plans.

Collaboration & Communication :

  • Communicate regularly with supervisors and the healthcare team regarding patient progress, issues, and plans.
  • Participate in patient care conferences, utilization review meetings, and rehabilitation conferences as needed.
  • Instruct patients, families, or nursing staff on maintenance programs and caregiver training for discharge.
  • Contribute to discharge planning.
  • Supervision & Training :

  • Supervise Physical Therapy Assistants (PTAs) in direct patient care and related activities, adhering to state practice acts.
  • Train nursing staff on restorative aide techniques.
  • Oversee PT aides and students.
  • Documentation & Compliance :
  • Maintain accurate and timely documentation, including evaluations, daily notes, weekly progress notes, and recertification records, in compliance with state practice acts and payer requirements.
  • Record treatment changes according to policy.
  • Equipment & Facility Management :

  • Recommend and facilitate ordering of durable medical equipment (DME) such as braces, walkers, canes, elevated toilet seats, and grab bars to enhance patient independence.
  • Report any equipment malfunctions promptly to ensure proper working order.
  • Professional Conduct :

  • Foster positive and effective relationships with patients, residents, families, staff, and professional colleagues.
  • Participate in in-service training for other facility staff.
  • Knowledge, Skills, and Abilities

  • Computer Proficiency : Proficient in data entry and using healthcare software (e.g., Casamba, Point Click Care).
  • Analytical Skills : Ability to interpret technical procedures, policy manuals, and apply mathematical concepts (fractions, percentages, ratios) to practical situations.
  • Problem-Solving : Skilled in solving practical problems and interpreting various instructions (written, oral, diagram, schedule).
  • Communication : Excellent ability to present information and respond to questions from managers, coworkers, and families.
  • Education & Experience

  • Required : Bachelor's Degree in Physical Therapy. Master's or Doctorate preferred.
  • Licensure : Current and active Physical Therapist license in good standing with the state board.
  • Continuing Education : Must meet all state continuing education requirements.
