Cancer Care Navigator RN

Job Description :

The Oncology Nurse Navigator serves as a vital resource throughout the continuum of cancer care. This role requires knowledge on multiple treatment decisions, strong emotions, and financial impact of a cancer diagnosis. The Oncology Nurse Navigator will triage physical, psychological, and social needs of patients and their significant others, offering interventions and / or referrals as needed.

Key Responsibilities :

• Enable patients with information and education to reduce stress involved in a cancer diagnosis
• Empower patients to participate in their personal care process
• Improve understanding of decisions and treatment plans
• Complete treatment through survivorship planning (when applicable)
