We’re growing and looking for a Care Coordinator / Liaison with proven private pay home care experience to help expand our presence throughout the Greater Phoenix area .
Position Summary :
The Care Coordinator / Liaison serves as the face of Visiting Angels —conducting client consultations, developing individualized care plans, and fostering referral relationships within the community. This role requires a motivated, relationship-driven professional who understands the business side of private pay home care and is comfortable closing consultations with confidence and compassion.
Key Responsibilities :
- Conduct in-home care consultations and assessments for prospective clients and families
- Create and implement customized private pay care plans tailored to client needs
- Drive referral growth and community partnerships with hospitals, senior living communities, home health agencies, and social workers
- Execute marketing and outreach strategies to build brand awareness and increase inquiries
- Maintain ongoing client relationships through follow-ups and quality assurance visits
- Collaborate with caregivers and the office team to ensure consistent service excellence
Required Qualifications :
Minimum 2–3 years of experience in private pay home care or private duty services (required)Proven success in care coordination, client intake, or community liaison roles within home care, home health, or hospiceStrong understanding of non-medical home care operations, service agreements, and family dynamicsExcellent communication, presentation, and follow-up skillsValid driver’s license and reliable transportation (local travel requiredPreferred Skills :
Experience conducting in-home assessments and closing consultationsEstablished connections with referral partners in the Greater Phoenix areaWorking knowledge of CRM or home care software (AxisCare, ClearCare, or similar)What We Offer :
