Hospital experience is required if ARU experience is not available.

Description :

We are seeking a Case Manager for our rehab division.

The selected candidate will be responsible for managing patient care in a rehabilitation setting.

The Case Manager will work on a day shift schedule from 08 : 00 to 16 : 30.

The primary role includes coordinating care, facilitating discharge planning, and collaborating with healthcare teams.

This position offers the opportunity to float to other units if needed, though not regularly scheduled.

A strong commitment to patient advocacy and management is essential for this role.

Candidates should have effective communication skills and the ability to work in a team-oriented environment.

This position requires a candidate with a strong attention to detail and the ability to work under pressure.
