Registered Nurse Case Manager

Overview\nWe are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. In this role, you will be responsible for coordinating patient care, ensuring that individuals receive the necessary support and services tailored to their unique needs. • Develop and implement individualised care plans in collaboration with healthcare providers.\n
• Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.\n
• Provide education and support to patients and their families regarding health management and available resources.\n
• Coordinate services across various settings including senior care, hospice care, and home care.\n
• Supervise nursing staff and manage team performance to ensure high-quality patient care.\n
• Maintain accurate documentation of patient interactions, assessments, and interventions.\n
• Ensure compliance with healthcare regulations and organisational policies.\n
• Collaborate with interdisciplinary teams to enhance patient care delivery.\n\nSkills\nThe ideal candidate will possess a diverse skill set including : \n
• Experience in assisted living, nursing homes, senior care, hospice care, and home care settings.\n
• Strong background in case management with a focus on patient-centred approaches.\n
• Proficiency in managing patients with dementia, Alzheimer's disease, disabilities, or those requiring toddler care.\n
• Knowledge of anatomy and experience in ICU or laboratory settings.\n
• Expertise in medication administration and medical imaging techniques.\n
• Exceptional supervisory skills with experience in team management.\n
• Strong communication skills for effective collaboration with healthcare professionals and families.\n
