We are seeking a Transition Specialist to join our GME program at BayCare!

Position Details :

  • Location : Tampa, FL
  • Status : Full Time (non-exempt)
  • Shift : 8 : 00 AM to 5 : 00 PM Monday through Friday

    Responsibilities :

  • Manages the updates for the transitions roster and schedule.
  • Onboarding status spreadsheet, creation and distribution of the transition workplans, and communications with various departments to manage the workflow of system integration for provider onboarding.
  • Tracking, monitoring and troubleshooting the progression of the providers as they onboard in the medical group through various system builds in preparation for their start.
  • Creating and updating various reporting as needed by the department.
  • Provides additional departmental support in other areas or on-site transitions as needed.

    • Minimum Qualifications :

  • Required : High School Graduate or Equivalent GED with 3 years practice operations or related health care OR Associate's Degree with 1 year practice operations or related Health Care.
