We are currently hiring a full time Intake Coordinator - Licensed Vocational Nurse (LVN) / Licensed Psychiatric Tech (LPT) for Fremont Hospital !

Evening shift available :

PM (3pm - 11 : 30pm)

As an active member of the Assessment & Referral Team, the Intake Coordinator - LVN / LPT performs the Initial Psychiatric Screening Assessments for incoming patients and coordinates with the Medical Staff in directing the patient towards an appropriate level of care.

Fremont Hospital is a fully accredited, 148-bed private inpatient psychiatric hospital providing quality care for children through adults. The facility provides a full assortment of care which includes assessments, inpatient psychiatric hospitalization, partial-hospitalization, intensive outpatient psychiatric treatment as well as chemical dependency outpatient care.

Qualifications

Education : AA / AS Degree or Bachelor's Level Degree preferred; Master's in social work, psychology or related mental health field, PhD and PsyD candidates in terminal degree programs with Masters equivalent or a degree from an accredited program in nursing preferred.

Experience : A minimum of one (1) year direct clinical experience in a psychiatric or mental health setting, with direct experience in patient assessment, family motivation, treatment planning, and communication with external review organizations.

Licensure : LVN or LPT licensure required.
