The OT will provide rehabilitation services in a skilled nursing / rehab setting, focusing on patients with cognitive and / or ADL physical functioning disorders. This role emphasizes evidence-based, patient-centered care to improve independence and functional outcomes. Responsibilities Screen and evaluate patients with cognitive and / or ADL physical functioning disorders Develop and implement individualized treatment plans following regulatory and clinical standards Provide skilled, medically necessary interventions including ADL retraining, therapeutic activities, exercises, modalities, and use of assistive devices Assess patient progress and adjust treatments as needed, documenting in the electronic health record Evaluate treatment outcomes and modify services according to patient needs Consult and counsel patients, families, and caregivers regarding therapy plans Communicate effectively with supervisors and interdisciplinary team members regarding patient progress and care plans Complete comprehensive discharge summaries and maintain accurate patient records Accurately record treatment minutes for all provided services Participate in meetings, in-services, and facility trainings Obtain necessary DME or supplies as needed Supervise and support OT assistants, rehab aides, or students Adhere to NBCOT, AOTA, and state professional practice acts, codes of conduct, and ethical standards Requirements Minimum 1 year OT experience – Required New grads accepted with Level II fieldwork experience LTC or Rehab experience – Required Active OT license – Required Contact HumanEdge Health / >
