Responsibilities Work in a home health setting as a Case Manager Manage an assigned caseload of patients in the community Day shift schedule, Monday through Friday, 8 : 00 AM to 4 : 30 PM Utilizes Epic for charting; prior Epic experience preferred Coordinate and manage comprehensive care and services for each patient in your caseload Oversee care planning, visit productivity, and expense control Collaborate with the interdisciplinary team to implement care plans May assist in orientation, competency evaluation, and staff consultation as needed Only one holiday time off request will be approved during the assignment Must complete orientation modules through e-learning, with module hours varying by specialty Return staff must have been separated from the facility for at least 1 year before reapplying as a traveler Must be available to complete required lab work within 3-5 days of acceptance Requirements Active CA RN license required Current BLS certification from AHA or American Red Cross required Minimum of 2 years' home health case manager experience required Must provide 2 recent professional references Must reside at least 50 miles from the facility to qualify for travel pay Prior Epic EMR experience is preferred Benefits High Pay and Bonuses Medical, Dental and Vision Insurance with Day 1 Options 401(k) Plan Weekly Payroll Deposit Free Online CEUs Generous Housing Allowance Travel and Licensure Reimbursements Non-taxed Per Diem and Subsidy Traveler Rewards and Discounts American Traveler Job ID P-619378.
