Key Responsibilities

Provide support for clinical outcome management and healthy lifestyle information to patients

Complete transitions of care calls for moderate risk patients and document thoroughly

Assist patients with health literacy, medication instructions, and follow up on care plans

Required Qualifications

Certificate Program Post High School Vocational / Specialized Training Required

1 year of healthcare experience required

Licensed Practical Nurse or Multi-State License upon hire required

Basic Life Support certification upon hire required

Completion of all required education programs and competency testing upon hire required
