The role
The Hospice Chaplain is a qualified individual that provides guidance and support to other team members to ensure the spiritual needs and concerns of the hospice patients and family are met via the development of a patient and family’s plan of care.
Key Responsibilities :
- Provide spiritual and bereavement counseling to the hospice patients and family and assist the patient and family in evaluating his / her spiritual and bereavement needs
- Be an active participant in interdisciplinary group meetings and maintain an open communication with all members of the interdisciplinary group
- Maintain strict patient confidentiality
- Facilitates connection to spiritual resources in the community as needed
- Preside over funeral services as requested by the family
- Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care
Skills and Experience Required :
Bachelor’s degree in religious studies, social work, psychology, counseling, pastoral education, or related fieldMust maintain a valid driver’s license and good driving recordAt least one (1) year experience within hospiceDocumented evidence of training and experience in dealing with grief and loss counselingBe a recognized spiritual leader known throughout the communityBe able to affirm the religious and spiritual freedom of all persons and refrain from imposing doctrinal positions or spiritual practices on persons whom they encounter in their professional role as chaplainBe available at all times during agency operating hours or as needs ariseHas the mental and physical health necessary to meet the demands of the positionExcellent written and verbal communication skillsStrong leadership and organizational skillsProficiency in personal computer use including email, clinical, word processing, and spreadsheet softwarePreferred Knowledge, Skills and Experience :
Master’s degreeCompletion of at least 1 Clinical Pastoral Education (CPE) unitCertified in Thanatology : Death, Dying and Bereavement (CT), through ADECTwo years of experience with grief and loss counselingElectronic Medical Records experience
