Terrace Health and Rehabilitation Center  is seeking a dedicated and enthusiastic  Physical Therapist (PT)  to join our team full-time. In this rewarding role, you will provide comprehensive rehabilitation services to residents in our subacute rehabilitation unit, helping them regain mobility and enhance their quality of life.

Key Responsibilities :

• Patient Assessment :  Conduct thorough evaluations to assess patients’ physical capabilities, limitations, and rehabilitation needs.
• Individualized Treatment Plans :  Develop and implement tailored treatment plans based on each resident's specific goals and needs.
• Therapeutic Interventions :  Utilize a variety of therapeutic techniques, exercises, and modalities to improve strength, mobility, and functional independence.
• Patient Education :  Educate residents and their families about rehabilitation processes, exercises, and strategies for preventing injuries.
• Collaboration :  Work closely with a multidisciplinary team, including occupational therapists, speech-language pathologists, nurses, and physicians, to provide comprehensive patient care.
• Progress Monitoring :  Regularly evaluate patient progress and modify treatment plans as necessary to ensure optimal outcomes.
• Documentation :  Maintain accurate and timely documentation of evaluations, treatment plans, and progress notes in accordance with regulatory standards.
