Position Summary :

The Licensed Behavioral Health Consultant (BHC) provides behavioral health services within a primary care setting as part of an interdisciplinary team. The BHC is responsible for conducting behavioral assessments, delivering brief therapeutic interventions, and collaborating with medical providers to support patients behavioral and emotional well-being. This position plays a key role in enhancing patient outcomes through integrated, coordinated care.

Essential Duties and Responsibilities :

• Conduct behavioral health assessments and screenings for patients referred by medical providers.
• Provide brief, evidence-based interventions for behavioral health conditions such as depression, anxiety, trauma, and stress-related concerns.
• Collaborate with primary care and clinical teams to develop and implement coordinated care plans.
• Deliver psychoeducation and skill-building strategies to support patient engagement and self-management.
• Maintain timely and accurate clinical documentation in compliance with organizational, state, and federal standards.
• Participate in interdisciplinary case consultations, staff meetings, and professional development activities.
• Contribute to continuous quality improvement and performance initiatives within the behavioral health program.
• Work with patients of all ages.
