Job Summary
The Referral Coordinator ensures timely and accurate referral processes for patients requiring specialty care. This role involves scheduling, documentation, and communication between patients, primary care providers, specialists, and insurance companies. You'll be essential in helping patients navigate the healthcare system while supporting our commitment to safe, coordinated care.
Key Responsibilities
- Track and document referral progress to ensure completion and patient safety
- Register patients accurately, capturing current demographic and insurance information
- Gather and organize relevant clinical documentation to support referral needs
- Communicate with specialists to transmit appropriate medical records
- Confirm insurance authorizations and prior approvals as needed
- Collaborate with financial services to maximize billing and reimbursement
- Educate patients on referral details and what to expect from the process
