Position Overview :

We are seeking a dedicated and compassionate full-time Registered Nurse Case Manager to join our healthcare team. In this rewarding role, you will coordinate and oversee the individualized care of patients, ensuring they receive the highest standard of medical services in their home environment. Key Responsibilities :

• Coordinate and manage the comprehensive care of assigned patients.
• Develop, implement, and regularly update individualized care plans.
• Perform skilled nursing assessments and provide direct patient care.
• Collaborate with physicians, therapists, social workers, and other healthcare professionals.
• Educate patients and families on disease processes, medications, treatments, and care plans.
• Ensure timely and accurate documentation of all care activities.
• Participate in case conferences and interdisciplinary team meetings.
• Serve as a compassionate resource for patients and families, providing emotional support and guidance.
