Program Description
Cascadia operates three secure residential homes in the Portland Metro area. These are homes for adults who experience severe and persistent mental health challenges. Employees work as a team to provide comprehensive services that focus on psychiatric treatment, personal care, skills training, and case management.
Position Description
The Medication Services Coordinator is responsible for medication room related services at the assigned facility. This includes providing day to day provision of medication administration, PCP care coordination services, as well as quality assurance and oversight of the medication services performed on site.
Essential Responsibilities
- Manage the administration, documentation, and evaluation of the medication management program.
- Oversee medication administration room, including inventory, supply orders, tools, and materials.
- Conduct periodic audits of medication room services to ensure quality of service, and documentation compliance with all applicable internal and external regulations.
- Maintain timely and effective coordination and communication with site medical staff.
- Provide leadership and primary oversight for MAR completion, 'Cycle Fill' documentation / ordering, pharmacy coordination and communication, as well as narcotic medication handling procedures.
- Schedule medical services appointments, with primary care providers, specialists, dental services, and hospital / E / D's.
- Coordinate with LMP, and clinical leadership for effective provision of self-administered medication education and skills building curriculum for all residents.
- Facilitate program goals and development, while meeting productivity and program expectations, and operating within the program budget.
- Collaborate with program leadership and applicable community partners to develop and implement the regulations, policies, and procedures under which the program operates.
- Maintain knowledge of, ensure compliance with, and review all applicable federal, state, county, and city regulations and approved internal policies and procedures.
- Document all services in a professional manner using the agency EMR system.
- Carry partial or total responsibility for developing and implementing assigned operational functions within the medication room.
- Provide positive role modeling and mentoring to peers while fostering a client centered approach by developing, maintaining, and advancing the consistent use of clinical best practices.
- Participate in program development and trainings.
- Provide input on programmatic documentation / procedural requirements.
- Provide oversight and continuity of care, as well as communication and task assignment during all staff / team meetings, shift changes, and related procedures.
- Provide input toward the therapeutic treatment plan in collaboration with the case manager.
- Complete annual employee training requirements on a timely basis as indicated in the licensing requirements for the facility as well as agency requirements.
- Perform other duties as assigned.
Qualifications Education
Bachelor's degree in a behavioral science field from an accredited college or university or three years of full time professional experience working with adults with severe and persistent mental illness.
Experience
Experience working with adults with severe and persistent mental illness preferred.

Possess a valid driver's license, three year driving history; clean driving record, ability to pass Driver Motor Vehicle (DMV) check and complete the Cascadia online Defensive Driving course.
Working Conditions
Work will be performed in a secure (locked) residential setting. The nature of the work dictates that the employee is alert and attentive during his or her assigned shift.

Physical Demands
Activities and structure support community integration assist program residents in learning socially acceptable behaviors.
