Position : ED Clinical Social Worker- LCSW- 24 Hrs Evenings at Boston Medical Center (BMC)
Overview
Utilizing clinical social work techniques and theory, consults on, assesses, and intervenes with high-risk patients and their families regarding the impact / amelioration of emotional and social problems. Duties include psychosocial assessments, psychosocial diagnosis, planning and implementing effective treatments, evaluation, education, supervision, coaching and mentoring of the patient and family members. The position consults with, collaborates and communicates with a wide range of social, governmental and legal agencies, courts, schools, clinics, other hospitals, physicians and other sources in the overall care of the patient.
Responsibilities
- Assesses patient and family to ascertain emotional and social problems using the department's psychosocial assessment tool as a guideline.
- Identifies issues to be addressed during the patient's care and formulates the plan.
- Completes biopsychosocial assessments and other specialized trauma assessments.
- Provides trauma-informed, strengths-based interventions for patients and families.
- Communicates effectively with providers, nurses and other healthcare professionals and team members in the Emergency Department.
- Initiates and leads family meetings when facing catastrophic news : disease progression, traumatic incidents, loss or bereavement.
- Supports patients and families in crisis; initiates family meetings with ED providers.
- Interacts independently with providers, nurses, and ED healthcare professionals on behalf of patients and families.
- Responds promptly to P1 and P2 traumas and assists the team in identifying patients and next of kin.
- Provides case management and advocacy for patients and families.
- Collates with multi-disciplinary teams and community providers to address patient and family needs.
- Assesses safety risks and provides brief, solution-focused interventions and safety plans as needed.
- Uses motivational interviewing, reflective listening, and trauma-focused cognitive behavioral techniques to support patients and families.
- Provides crisis intervention and problem-solving support to patients and families in crisis.
- Offers information, referrals, and counseling related to health issues, child protection, family violence, illness, community violence, substance use / abuse, disabilities, mental health, bereavement and family homelessness.
- Collaborates with BMC programs (e.g., VIAP, CVRT, Safety & Support, Elders Living at Home) and with community agencies (e.g., BPD, BHA, BMH, Elder Services, DCPD, DMH, DDS, DHCD, Mass Rehab Commission / SHIP).
- Responds promptly to consult requests and referrals for social work supports; educates ED staff as needed.
- Triages cases and prioritizes time-sensitive needs; maintains ongoing communication with ED colleagues and Social Work Manager on call.
- Provides education, crisis intervention, and emotional support to patients, families, and ED staff as needed.
- Develops and maintains therapeutic relationships with patients and families.
- Screens and assesses for social determinants of health; reports suspected abuse / neglect and ensures timely comprehensive consultations with legal mandates.
- Involves patient's family when appropriate and contacts family the same day if indicated.
- Collaborates with healthcare team to ensure emotional and social needs are addressed; provides psychotherapeutic treatment and psychosocial support.
- Maintains patient confidentiality and documents each encounter in the medical / department records, with date, time, title and signature.
- Works independently, initiates interventions, and triages in busy ED conditions; provides clinical supervision to staff and interns as assigned.
- Complies with documentation, regulatory and professional requirements; submits department statistics and reports within required time frames.
- Maintains competency to ensure quality care and participates in staff meetings, supervision, and continuing education; supports graduate and undergraduate training programs as assigned.
- Identifies operational weaknesses and recommends improvements; represents the department within the hospital and the community.
- Maintains a neat and orderly work area and adheres to emergency procedures and hospital conduct standards.
Qualifications
Master's Degree from an accredited school of Social Work with experience in trauma-informed care.Massachusetts Licensed Certified Social Worker (LCSW) required.Previous experience with children and families, child protection, and child welfare required.Crisis experience required; experience with vulnerable populations (e.g., persons with disabilities, elders, substance use disorders, homelessness, new migrants, victims of violence, mental health concerns) preferred.Hospital or healthcare setting experience strongly preferred.Knowledge of social work process and community resources related to health, illness, and disability (1–2 years on-the-job preferred).Knowledge and skills : community resources for families, advanced interpersonal skills, ability to work in an interdisciplinary team, strong written and verbal communication, cultural sensitivity, and computer literacy (MS Word, Excel, Outlook).Equal Opportunity Employer / Disabled / Veterans
