Bachelor of Science in Nursing (BSN) degree, active Nurse Practitioner (NP) license and minimum 1+ years Nurse Practitioner (NP) experience required. Applicants who do not meet these qualifications will not be considered.

  • Are you a Nurse Practitioner seeking a flexible schedule and the opportunity to make a difference in patients' lives?
  • Sunbelt Staffing is seeking a Per Diem NP to conduct home health risk assessments for Medicare Advantage plan members in Albany, NY and surrounding AlbanyCounty. (Cohoes, Watervilet, and Westmere)
  • This is a great opportunity for a compassionate and independent NP to work on-site with patients in the comfort of their homes.

Responsibilities :

  • Conduct comprehensive annual home health risk assessments (45 minutes average).
  • Evaluate patients' medical history, conditions, and medications.
  • Educate patients on health conditions and medications.
  • Identify gaps in care and provide recommendations.
  • No creation of treatment plans or prescribing medications required.

    • Work Schedule :

  • Flexible! Create your own schedule with shifts available Monday-Sunday.
  • Minimum weekly or bi-weekly hourly commitment.

    • Qualifications :

  • NY Nurse Practitioner License.
  • Current CPR certification (BLS).
  • Preferred : 6 months of current NP experience.
  • Preferred : Board Certification through AANP or ANCC.
  • MSN degree from an accredited school of nursing.
