Position Overview
The
LTSS Service Care Manager
provides field-based, member-facing care coordination and case management services to members enrolled in the Long-Term Services and Supports (LTSS) program. This role ensures that members receive comprehensive, person-centered care through assessments, follow-ups, and collaboration with interdisciplinary teams.
Key Responsibilities
Conduct biannual member visits and document findings in accordance with program standards.
Complete WLI / tasks as assigned and maintain compliance with our documentation requirements.
Coordinate care with multi-disciplinary healthcare teams to support members’ individualized care plans.
Perform follow-up calls with members to ensure ongoing support and service delivery.
Participate in additional LTSS or special projects as assigned.
Engage in daily check-ins with Team Lead and Manager; complete training and shadowing of other Service Coordinators prior to independent fieldwork.
Education & Licensure
Required :
Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN)
Preferred :
Prior Managed Care Organization (MCO) experience
Case management and assessment experience

Bilingual – English & Spanish (required)
Familiarity with Microsoft Outlook, Excel, and Teams
Experience performing member assessments and case management
Nice-to-Haves :
Previous experience in managed care or LTSS programs
