This role fosters a "One Team" approach, ensuring collaboration among medical, clinical, and operations teams to achieve the highest quality of care and support. Key responsibilities include travelling to homes, providing the highest quality of care to include medication management, medical treatment needs, education, and providing support to the Direct Service Providers.\n\nSupervisory Responsibilities : \n\nSupervises Licensed Practical Nurses and Certified Nursing Assistants as needed per market.\n\nPrimary Responsibilities : \n

  • Always be compliant with all company and regulatory policies and procedures.\n
  • Weekly and daily follow-up with to monitor infection(s) when needed.\n
  • Administration of medications and vaccinations as requested by the physician or psychiatrist.\n
  • Assess individuals upon admission and per state requirements thereafter meeting the needs of the individuals served.\n
  • Under the direction of the DON, identifies, coordinates and monitors the health care of the individuals served, especially those with complex medical needs at assigned sites.\n
  • Follows the verbal order procedure.\n
  • Administration of first aid as needed.\n
  • Provides care as ordered by physicians.\n
  • Diabetic foot assessments.\n
  • Assists with coordination of care and treatment for the individual served to ensure psychiatric, medical, dental, and optical appointments are scheduled as needed.\n
  • Communicates and coordinates and physical health concerns and follow up treatment with physicians and psychiatrists.\n
  • Performs medication administration and initial medical classroom training and ensure state requirements are being met.\n
  • Educate Direct Support Professionals (DSP) on the use of scales, blood pressure, oximeter, thermometer, stethoscope, glucometer, wheelchair, and any other durable medical equipment or any other need that is new to the home that is ordered by a physician.\n
  • Participates in medical on call rotation if required.\n
  • Assists with development and revisions of medical policies and procedures.\n
  • Assists in the destruction of controlled substances as outlined by DEA guidelines.\n
  • Responsible for triaging medical concerns.\n
  • Audit the medication room, EMAR and EHR programs following state requirements.\n
  • Assists in care planning as required per market.\n
  • Cooperates with Beacon Specialized Living and other entities during any inspection, or investigation, or accreditation reviews as requested.\n
  • Educates staff regarding infection prevention and monitoring and overall physical health of individuals served.\n
  • When needed, provide input / data / summaries as part of the Infection Control Plan.\n
  • Participate in the influenza campaign and provide data where requested.\n
  • Performs other duties as assigned or requested.\n
  • Adheres to BSLS Core Values, Mission Statement, and Corporate Philosophies.\n\nEducation and Qualifications : \n
  • Valid RN license (Missouri) with a minimum of two (2) years previous experience preferred.\n
  • Must pass criminal background check\n
  • Must possess a valid Driver's License.\n
  • Proficient in speaking, reading and writing the English language.\n
  • Approved by state, federal and government entities to work within BSLS programs.\n
  • Ability to use office equipment, including BSLS information technology systems.\n\nSkills and Abilities : \n
  • General knowledge of nursing and healthcare terminology.\n
  • Proven capability of communicating with multiple disciplines.\n
  • Proven ability to deescalate difficult situations.\n
  • Proven ability to be able to provide care as needed.\n
  • Excellent computer skills.\n

Universal precautions are required to minimize the risk of infections.\n\nWork sites may include locations that are not barrier-free. Duties are performed in an environment where there is a potential for physical aggression from individuals.\n\nPhysical Demands : \n\nThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.\n\nWhile performing the duties of this job, the employee is regularly required to talk or hear. This would require the ability to lift files, open filing cabinets, and bend or stand on a stool as necessary.\n\nPosition Type / Expected Hours of Work : \n\nSchedule may vary depending on the needs of the home(s) / program(s) and other nursing support scheduling needs. There will also be a rotation for on-call duties.
