Presbyterian Homes & Services - Flagstone is seeking a Full time Admissions Coordinator for its team.

SCHEDULE / HOURS : Monday through Friday (regular business hours).

This is a full time benefit eligible position scheduled for 40 hours / week on site.

REPORTING STRUCTURE : The Admissions Coordinator reports directly to the Care Center Administrator. This position works closely with Campus Leadership Team members, Site Leaders, and Functional Work Group Members.

Responsibilities

The Admissions Nurse (LPN) is responsible for enhancing patient and resident satisfaction by facilitating the nursing admission process. This includes collaborating with RNs and other healthcare professionals to assess appropriateness of admissions, assisting with coverage determinations, coordinating care with the multidisciplinary team, supporting discharge planning, and participating in utilization reviews for various payer types. The LPN ensures smooth transitions and adherence to care standards, contributing to efficient and high-quality patient outcomes.

Pay Range

The estimated starting pay range for this role is $28.88 - $36.09 per hour. This good faith estimate is based upon PHSs typical starting pay range for this role. Actual pay range depends on many factors including each individual employees qualifications and market conditions and is subject to change.
