  • Job Summary : Integrate collaborative care planning for individual patients and implement nursing standards in the processes of care.

  • 3+ years CURRENT MS - Required
  • EPIC EMR experience - HIGHLY Preferred
  • Previous Travel - Preferred
  • Traveler must have their own vehicle for this need
