A company is looking for a Care Coordinator in the Mountain Time Zone.

Key Responsibilities

Engage and enroll patients through telephonic outreach, serving as their primary contact

Develop and implement comprehensive care plans in collaboration with the interdisciplinary care team

Advocate for patients by identifying barriers to care and connecting them with necessary resources

Required Qualifications

3 years of experience in healthcare coordination or a related field

Experience with a team-based approach to care

Strong knowledge of community resources

Ability to prioritize tasks and work effectively in a remote environment

Intermediate skills with MS Office Suite, including Outlook and Teams
