About The Position

License : Valid physical therapy license required

Must-Have : SNF experience preferred, Covid test required prior to start date

Description :

Completion bonus for completing 13 weeks.

Completion bonus for completing 26 weeks.

Ensure prescreen is filled out and attached.

Resume and license verification need to be attached.

Failure to complete and attach required items will result in candidate's decline.

Candidates should be aware of credentialing checklist requirements.

Covid test must be conducted 72 hours prior to start date with results issued 48 hours prior to start date.

Candidates must provide references (3).

Criminal background checks will be conducted including county criminal (7 years), federal criminal (7 years), multi-state criminal (7 years), and nationwide sex offender search (7 years).

Social Security Number trace is required.

Candidates will be required to provide proof of immunization or testing for tuberculosis.
