• Conduct thorough assessments of patients and their families to determine home care needs\n
• Provide comprehensive physical assessments and document the history of illnesses\n
• Develop and update individualized care plans with active participation from patients and their families\n
• Identify discharge planning needs and ensure they are addressed before patient discharge\n
• Offer counseling to patients and families to address nursing and related health needs\nQualifications : \n
• Current and valid state RN license.\n
• Minimum of 1 year of recent acute care experience in an institutional setting.\n
• Basic Life Support (BLS), with hands-on CPR certification.\n
• Valid driver's license and auto insurance.\n
