RN Nurse Assessor / Case Manager (BSN)

Full Time

Salary : $94,662 / yr

If you want to be part of the only safety net hospital in Nassau County, we have an amazing opportunity for a full-time Nurse Assessor / Case Manager. You will work at the Nassau County Department of Social Services, performing UAS assessments and case management for Medicaid recipients who live in Nassau County and are seeking / receiving home care services.

We require :

• Registered Nurse with a Bachelor of Science (Nursing preferred)
• UAS Certification
• NYS Driver’s License is required, and you must be willing to drive anywhere in Nassau County and comfortable performing the UAS assessment in the patient’s home, at a hospital / nursing home setting, or by telephone
• At least two years of experience in home care
• Strong computer skills, must be comfortable performing the UAS using an iPad
• Bilingual Spanish preferred
