1. Completed US residency.

2. A valid unrestricted DEA with schedule II V.

3. Not had a lapse in practice >

six months leading up to assignment at CCHCF. (MUST provide phone numbers, contact names and dates of affiliations with all hospital(s) / clinic(s) in the 12 months).

4. Not had loss of licensure or suspended licensure of ANY medical profession.

5. Any lawsuits or board actions, please enumerate.

6. Any history of drug abuse, i.e. alcohol, narcotics, amphetamines, etc.
