Under the general supervision of the Medical Clinic Manager (MCM), the Licensed Vocational Nurse (LVN) plays a key role in supporting the delivery of high-quality, patient-centered care at Serve the People. The LVN is responsible for assisting with patient triage and working collaboratively with registered nurses and physicians to deliver comprehensive healthcare services across a variety of clinical settings. This position requires a compassionate, detail-oriented healthcare professional who is committed to supporting the health and well-being of our diverse patient population. The LVN contributes to a positive patient experience by ensuring efficient clinical workflows, maintaining clear communication, and upholding the highest standards of care. - Provide direct patient care by administering medication, monitoring vital signs, dressing wounds, colleting samples and assisting with personal hygiene needs.
- Assessing patients immediately upon arrival and priority-rating them based on the severity of their condition.
- Regularly reassessing patients throughout their stay to ensure that their condition is not deteriorating.
- Keeping track of waiting times and working to keep them as short as possible.
- Providing clear and concise instructions to patients about what they need to do next.
- Documenting all aspects of patient care in a clear and accurate manner.
- Collaborate with registered nurses and physicians to develop individualized treatment plans for patients considering their medical conditions, needs and preferences.
- Maintain accurate and up-to-date patient records including medical histories, symptoms, treatments and medications administered ensuring compliance with legal and ethical standards.
- Educate patients and their families on various health conditions, medications, treatments procedures and self-care practices to promote wellness and effective disease management.
- Assist healthcare providers during medical procedures, examinations and surgeries ensuring patient comfort, safety and adherence to sterile techniques.
- Continuously assess patient progress, observe any changes in their condition and report finding to the registered nurse or physician promptly.
- Communicate effectively with interdisciplinary healthcare team members, patients and their families to ensure coordinated and holistic patient care.
- Respond efficiently to medical emergencies, applying first aid and basic life support techniques as necessary and promptly notifying appropriate healthcare professionals.
- Adhere to established policies, procedures and healthcare regulations maintaining patient confidentiality and privacy at all times.
- Stay updated on advancements in nursing practices, attend educational workshops and participate in professional development activities to enhance your knowledge and skills.
- Adhere to HIPAA regulations and other relevant laws to protect patient privacy and confidentiality in all communications.
- Perform other duties as assigned by the executive leadership and administration.
Requirements :
At least two (2) years of clinical practice requiredExperience in a community health center or public health preferredPrior experience with an electronic matching system is a bonus.Ability to flourish in a team management systemExperience with electronic health records preferred.Sensitivity to low income, ethnic minority community.Critical thinkingExcellent leadership, coaching, and strong computer skills.Interacts with colleagues, team, and all staff in a professional and collegial mannerActive License Vocational Nurse in CaliforniaBLS CertificationCPR certification
