About The Position

Job Title : Registered Nurse

Must-Have : Proficiency in OASIS and HomeCare HomeBase EMR

Description :

Provides coordination and direction for the provision of home nursing care according to physician orders.

Utilizes agency policies and procedures through the competent application of the nursing process.

Reports to the Clinical Manager and directly supervises Home Health Aides and Licensed Practical Nurses.

Engages in home visits and participates in periodic office meetings.

Must have reliable transportation, a current driver’s license, and agency-required liability insurance.

Average patient load is 5-8 patients per day.

Each visit and activity holds a different point value, totaling around 30 points per week.

Types of cases include Start of Care, Wound Care, and Follow-up Visits.

Proficiency in HomeCare HomeBase is required, along with familiarity with Oasis.

Home visits may occur within a radius of up to 75 miles.

Millage is compensated at the IRS standard rate.

Documentation must be completed within 24 hours of the appointment.

Flexibility to accept all assigned patients regardless of distance is expected.

Open to on-call rotations, including monthly and holidays.

Local candidates are accepted under the same conditions.

There are no guaranteed hours, and creativity in meeting weekly productivity points is essential.
