Salary : $25-$33 an hour DOE

Licensed Vocational Nurse - LVN

This position reports to the Medical Director

MINIMUM QUALIFICATIONS :

Must possess a current License as a Vocational Nurse with the state of California.

Qualifications, Education, and Experience :

  • Graduation from an approved nursing program with degree, being associate degree or equivalent degree.
  • Must have knowledge through practical experience and / or training in the areas of
  • Must be aggressive in developing of client care programs within the clinical policies and program objectives and relating them to the funding requirements.
  • Must be sensitive to the needs of the clients in a community health care center setting, in the areas of health and social services as one component.
  • Possesses current valid California Driver License
  • Clean driving record
  • Knowledge of recovery programs
  • Working knowledge of the Microsoft Suite (Word, Excel, PowerPoint, Outlook)
  • Experience in related job and working in an office environment
  • Discretion and trustworthiness due to frequent contact with confidential material
  • Physical stamina and mobility including ability to reach, kneel and bend ability to lift, push and pull required load (usually about 30lbs)
  • Able to repeatedly bend at the waist and neck, twist and rotate hands, wrists, shoulders, elbows, and forearms.
  • Good communication skills.

  • Knowledge of MORs documentation
  • Knowledge of documentation requirements
  • Knowledge of waived testing practices
  • Knowledge of SUD assessment and treatment
  • Job Duties

  • Prepare an assessment documenting the reason for visit in the clients chart.
  • Observe and record client administration of vital signs, height and weight, etc.
  • Assess SUD history of clients.
  • Observe self-administration of medications by clients and document the observation in the client record.
  • Prepare and manage the detoxification watch schedule.
  • Update client charts in regards to allergies.
  • Educate and counsel clients concerning their disease, treatment, and prevention of disease.
  • Clean and sterilize office equipment.
  • Client screening.
  • Assist clients in making referral appointments.
  • Schedule HIV / Antibody Test Counseling with local provider in accordance with established policies and procedures and in compliance with State and Federal requirements.
  • Order office and UA supplies.
  • Participate in program development and case management.
  • Continually assess for areas of improvement and develop plans and evaluation criteria for such.
  • Schedule and transport clients for TB tests and other medically related appointments.
  • Develop and facilitate groups on diet, exercise, sleep hygiene, and the importance of compliance with their physicians orders.
  • Maintain all documentation required by DHCS and the Joint Commission.
  • Prepare reports to fulfill DHCS and Joint Commission requirements.
  • Participate in Quality Improvement as required.
  • Other duties as required or assigned by the clinical staff from time to time or when it is necessary.
  • Ability to relate to the public regardless of ethnic, religious and economic status.
  • Document, inventory, and secure client medications to remain compliant with DHCS standards.
  • Prepare medication observation records for each client that has medications.
  • Prepare a treatment plan that addresses any identified client needs in ASAM Dimensions 1 and 2.
  • Review client treatment plan according to DHCS standards.
  • Assist Case Managers with client discharge.
  • Oversee provision of IMS services in accordance with DHCS standards and company policies.
  • Conduct physical observations of clients and document evidence of the observation in accordance with company IMS policies and DHCS standards.
  • Must be trained in the delivery of detoxification services, identification of the signs and symptoms of medical distress / overdose, and all associated policies related to the provision of detoxification and IMS services.
