We are currently seeking a caring and experienced Hospice RN Case Manager to join our team to walk alongside as a supportive presence for patients and families beginning during their hospice journey.

If you're an experienced RN with a calling to serve and a desire to truly connect with patients and families, we’d love to welcome you to our team.

Responsibilities :

Care for patients where they call home.

Use your clinical skills and your emotional support skills to support patients and families.

Create care plans that treat the whole person—body, mind, and spirit.

Work closely with providers, referral sources, and our amazing interdisciplinary team (IDT).

Be a steady, calming presence during a difficult time—and help others feel just a little more at peace.

Educate families, empower caregivers, and be the go-to person when people have questions.

Requirements :

A current RN license in Michigan

Hospice, home care, acute care, or case management experience

Confidence working independently—but also a team-first attitude

On call needs based on YOUR schedule (nights, weekends, holidays)

A big heart and the ability to keep calm under pressure
