Job Overview\n\nThe Contractor shall provide professional Registered Nurse (RN) Care Management services on behalf of E-merge Resources. The RN Care Manager will play a key role in coordinating and delivering high-quality patient care, ensuring participants receive comprehensive and compassionate services across all care settings.\n\nThis position requires strong clinical judgment, effective communication skills, and a commitment to patient-centered care and advocacy.\n\nDuties and Responsibilities\n\nThe Contractor agrees to perform the following duties and responsibilities during the contract term : \n
- Conduct comprehensive patient assessments to identify medical, functional, and psychosocial needs.\n
- Develop and implement individualized care plans tailored to each participant's health goals.\n
- Coordinate and manage patient care across multiple settings, including inpatient, outpatient, and post-acute environments.\n
- Collaborate with multidisciplinary team members (IDT) to ensure seamless transitions of care and discharge planning.\n
- Monitor patient progress and revise care plans as necessary based on ongoing clinical evaluations.\n
- Educate and support patients and families regarding disease management, medication adherence, and community resources.\n
- Maintain accurate and compliant documentation per HIPAA and healthcare regulatory standards.\n
- Utilize critical thinking and clinical expertise to manage complex cases and facilitate positive outcomes.\n
- Participate in utilization management to ensure efficient use of healthcare resources while maintaining quality care standards.\n
- Provide limited local travel to patient homes when participants are unable to attend clinic visits.\n\nRequired Experience and Skills\n\nThe Contractor shall meet the following qualifications and competencies : \n
- Active Registered Nurse (RN) license in good standing with the State of North Carolina.\n
- Minimum of 2 years of nursing experience in one or more of the following : critical care, ICU, emergency department, geriatrics, behavioral health, medical-surgical, or outpatient care.\n
- Familiarity with Level I and Level II trauma settings preferred.\n
- Demonstrated experience in case management, discharge planning, and patient assessment.\n
- Knowledge of medical terminology, physiology, managed care, and utilization management principles.\n
- Proficiency in maintaining compliance with HIPAA and organizational documentation standards.\n
- Strong interpersonal, problem-solving, and communication skills with a focus on patient advocacy.\n\nContract Details\n
- Duration : 12 Weeks (with potential extension based on performance and client need)\n
- Compensation : $62.00 per hour\n
