Position Summary :

We are seeking a licensed and motivated Per Diem Occupational Therapist to join our rehabilitation team. This flexible, on-call position is ideal for a therapist who enjoys working in a collaborative, patient-focused environment and is available to cover shifts as needed, including weekends and holidays.

Key Responsibilities :

Conduct initial and ongoing assessments of residents’ physical and cognitive function.

Develop and implement personalized treatment plans to help residents improve their ability to perform daily living activities.

Provide direct occupational therapy services in accordance with physician orders and established care plans.

Collaborate with interdisciplinary team members to ensure cohesive and comprehensive care.

Document evaluations, progress notes, and discharge summaries in a timely and accurate manner.

Educate residents and caregivers on therapeutic techniques and safe use of adaptive equipment.

Qualifications :

Graduate of an accredited Occupational Therapy program.

Active Florida Occupational Therapist license in good standing.

Minimum of 1 year of experience in a skilled nursing facility or post-acute care setting preferred.

Ability to work independently and adapt to changing patient needs and schedules.

Strong communication, time management, and interpersonal skills.
