Nurse Aide
The Nurse Aide provides direct care for complex, acute, primary care, and long-term patients / residents per established Oklahoma City Veterans' Health Care System (OKC VAHCS) policies. The Nurse Aide will attend to needed and vital requirements of the patient / resident and ensure all regulatory and hospital guidelines and practices are followed, providing for a positive care experience within the OKC VAHCS.
Responsibilities include :
- Safely and accurately perform personal care skills, including measuring and recording vital signs to include temperature, pulse, respiration, blood pressure, and measuring of height and weight.
- Measures and records intake and outpatient, feeding techniques, non-sterile specimen collection, transfer, positioning, and turning techniques.
- Self-directed in seeking out RN, Lead Nurse, or Change Nurse to report patient-related information.
- Maintain appropriate communication with patients, families, and other health care workers, use medical asepsis and universal precautions, and documents care given is inherent to the role.
- Assists with patient education and documentation of education provided.
- Promotes independence of Veteran by means of respect, limit setting, and individualized treatment.
- Initiates basic life support and promptly recognize / respond to emergent situations.
- Offers supportive nursing care to critically or terminally ill patients / residents.
- Provides emotional support to patient / resident and / or family and significant other.
- Participates in unit base educational activities and continuing education opportunities to increase clinical and professional knowledge.
- Completes orientation, unit-based in-services, and mandatory educational activities in a self-directed manner by established timeframes.
- Identifies own deficits in skills and knowledge and acts to correct performance.
- Attend daily huddle and team meeting as per area.
- Functions as a therapeutic attendant for Veterans who require close observation or 1 : 1 observation.
- Prevents Veteran from hurting him / herself or interfering with the treatment by pulling out lines / IVs.
- Maintains direct supervision of the Veteran.
- Performs duties per policy to ensure documentation and safety are attained.
- Remotely monitor patients using provided equipment and technology.
- Uses the Electronic Medical Record, and other approved software as needed to perform duties in assigned area.
- Observes and reports patient complaints, results of treatments performed, change in patient's condition, behavior and / or attitude.
- Conserves resources effectively by using supplies and equipment in a responsible manner.
- Perform daily and / or weekly equipment / supply checks.
- Contributes to safe, clean, and therapeutic environment for patients and staff by maintaining clean equipment, patient's rooms and ward areas.
- Adheres to Safe Patient Handling practices for safe body mechanics, use of mechanical devices.
- Uses sound judgement when requesting assistance from other staff when moving, lifting, or turning patients to prevent injury to patient, self, or others.
- Demonstrates compliance with infection control practices, such as hand washing and wearing of appropriate personal protective equipment.
- Demonstrates safe work practices.
- Complies with safety rules and regulations for safe job performance and fire protection.
- Reports all accidents and reports changes to RN, Lead Nurse, Charge Nurse or Nursing Supervisor.
- Documents findings in patient / resident's medical record.
- Reports / responds appropriately to internal and external emergencies and disasters.
- Collaborates with peers in completing unit workload efficiently.
- Communicates information in a calm, non-judgmental, non-threatening manner.
- Adheres to established policies on patient / resident abuse, ethical conduct, and patient / resident rights.
