Key Responsibilities :

• Implement specific care plans for clients, documenting completed tasks accurately.
• Assist clients with personal care needs such as bathing and dressing.
• Help with light housekeeping, meal preparation, and laundry tasks.
• Provide transportation for clients to medical appointments, grocery shopping, and essential errands.
• Support clients in self-administering their medications.
• Monitor and report changes in clients' conditions promptly.
• Maintain confidentiality and handle sensitive information responsibly.
• Demonstrate consistent attendance and adapt to department needs.
• Comply with Medicare, Medicaid, and HIPAA regulations.
• Adhere to all organizational policies, procedures, and standards.
• Perform additional duties as assigned.

Requirements & Competencies :

With over 20 years of experience, Ambercare offers Home Health Care and Personal Care Services, ensuring individuals and families receive the support they need in their homes.
