Position Details :

Start Date : 11 / 24 / 2025

Location : Phoenix, AZ

Schedule : 3x12 Days

Contract Length : 13 weeks

Requirements :

  • Active Arizona PT license (no license hits)
  • BLS certification (AHA or Red Cross)
  • Minimum 2 years of recent Physical Therapy experience (inpatient preferred)
  • Travel experience preferred
  • Candidates with AZ home address on driver’s license are not eligible
  • Must meet Banner Health compliance and background requirements
  • Flu vaccination (or approved declination form)
