About The Position

Job Title : Physical Therapy Assistant

Job Specialty : Rehabilitation Therapy - Physical Therapy

Job Duration : 13 weeks

Shift : Monday-Friday, Days

Guaranteed Hours : 32 hours per week

Experience : New graduates are welcome, Skilled Nursing Facility (SNF) experience preferred

License : Relevant state license required

Certifications : 8 hours of dementia training

Must-Have :

  • Covid test 72 hours prior to start date with results issued 48 hours prior
  • Prescreen filled out and attached upon submission of the candidate
  • Resume and license verification must be attached if these items are not completed and attached, the candidate will be declined

Job Description :

  • Ensure prescreen is filled out and attached when submitting the candidate.
  • Attach a resume and license verification.
  • Covid test required 72 hours prior to start date with results issued 48 hours prior to start date.
  • Perform state license verification.
  • Provide three references.
  • Complete county criminal checks for the last 7 years for all counties lived and worked.
  • Conduct federal criminal search for 7 years or more.
  • Perform multi-state criminal search for 7 years or more.
  • Complete nationwide sex offender search for 7 years or more.
  • Conduct HHS / OIG search.
  • Conduct GSA search.
  • Provide social security number trace.
  • Provide 1 Step PPD or Quantiferon or proof of positive PPD and chest X-ray.
  • Conduct SAM search.
  • Complete CORI Massachusetts background check.
  • Complete 8 hours of dementia training.
