About the Role :

• Under the immediate supervision of the Clinical Manager, the Case Manager is responsible for each component of the client's treatment planning, client's behavioral health plan, including all programmatic implementation.
• Case managers will also coordinate aftercare plans such as MAT service appointments, mental health continuums, primary physician appointments, relapse prevention support within the community including NA / AA meetings and identify transportation for client at time of discharge. The Case Managers will also meet with their assigned case load individually every 7 days to adhere to documentation requirements through Medicaid standards.

Job Qualifications :

High School diploma preferred,

Minimum of 1-2 years of related experience or training

Ability to read, write, and comprehend the English language- Simple instructions, short correspondence, & memos

Positive demeanor and clear understanding of professional boundaries with clients

Ability to follow directives explicitly, adhere to facility policies, provide patient-driven customer service, and exercise time management skills.

Dependability, punctuality, accuracy, attention to detail, & thoroughness are all essential to this role.

Completion of CPR and CPI before first shift on the floor

Schedule flexibility, holiday, and weekend rotation is REQUIRED.

Must hold a valid driver's license with reliable transportation.

Compensation : $20-24
