Hospital Liaison – Skilled Nursing & Patient Transitions | Kingston of Ashland, OH

Full-Time | On-Site | Healthcare Sales & Relationship-Building Role

Kingston of Ashland is seeking a dynamic and compassionate Hospital Liaison to connect hospitals, families, and our skilled nursing and rehabilitation community in Ashland, Ohio . This is a rewarding opportunity to combine your communication skills, healthcare knowledge, and relationship-building expertise to improve patient transitions and outcomes.

What You’ll Do as a Hospital Liaison

Build Connections : Serve as the primary link between Kingston of Ashland and local hospitals, discharge planners, and families.

Guide Transitions : Conduct initial assessments, coordinate admissions, and ensure smooth transfers from hospital to skilled nursing care.

Educate & Inform : Share information with hospital staff and families about our services, rehabilitation programs, and resident-centered approach.

Support Families : Provide compassionate guidance, answer questions, and ease concerns throughout the transition process.

Drive Referrals : Identify opportunities to grow partnerships with healthcare providers and strengthen referral networks.

Stay Current : Maintain knowledge of industry best practices, discharge processes, and regulatory standards.

What You’ll Bring

Strong relationship-building and communication skills to connect with hospital staff, patients, and families

2+ years of experience in healthcare sales, marketing, admissions, or business development preferred

Knowledge of hospital discharge planning, insurance pre-certification, or case management a plus

Highly organized and detail-oriented, able to manage multiple referrals and transitions

Flexibility to meet hospital schedules, including occasional evenings and weekends

Clinical or healthcare background (RN, LPN, LSW) or healthcare-related customer service experience preferred but not required
