• Provide home-based care to patients with illnesses, injuries and chronic diseases—often after discharge from a hospital, rehab or nursing facility\n
• Work with a team of physicians and oversee LPNs, CNAs and aides providing patient care\n
• Examine patients, do assessments, document progress and report changes to their physician\n
• Educate patients on their plan of care, prescribed medication, therapy, diet and exercise\n
• Coach family members on the patient's plan of care, medication and home safety\n
• Ensure goals are met and coordinate discharge from services\n\nA few must-haves for Home Health Registered Nurses : \n
• Graduate of an accredited nursing program and active RN license in Virginia\n
• Minimum of 2 years of nursing experience, ideally in home healthcare\n
• CPR certification\n
• Knowledge of state and federal home health regulations\n
• Clinical proficiency in nursing specialty, strong communication skills and compassion\n
