Step-Down Navigator - Full-time

The Step-Down Navigator is responsible for meeting with all edlgible patients to discuss the benefits of Outpatient services as part of their continuum of care.

Responsibilities include but not limited to :

• Meeting with eligible patients.
• Consulting with Clinical Services, Utilization Management, and Outpatient on patient eligibility.
• Maintain current awareness of Outpatient programming.
• Identifies and communicates data driven trends in the step-down process.
• Assists in the development of strategies, goals and objectives related to the step-down process.
