Tufts Medical Center is a leading healthcare institution that is dedicated to providing exceptional care for transplant patients. Our Transplant Infectious Disease (ID) program plays a vital role in ensuring the best outcomes for patients undergoing solid organ transplant. Transplant ID is an integral part of the multidisciplinary services. Tufts performed the highest number of heart transplants in the Greater Boston area in 2024, and our program continues to grow in both volume and complexity. The transplant ID team includes ID physicians and an NP who collaborate with cardiology, nephrology, hepatology, surgical services, and hematology to manage all infectious disease aspects of care for pre- and post-transplant patients. We are looking for a LPN Navigator to join our team helping with transplant patient data management, laboratory tracking, coordination of patient appointments, schedules, direct patient calls, and help with care plans utilizing the nursing process and adhering to the standards of nursing practice. We are seeking a highly motivated and organized LPN to join our team and provide protocol-aligned clinical support and help strengthen our transplant outcomes and patient safety.
The Transplant ID LPN Navigator will serve as the primary point of contact for transplant patients regarding infectious disease management. This role involves coordinating care among the transplant ID team, multidisciplinary providers, and patients to ensure timely interventions, follow-up, and education. The LPN navigator will facilitate communication, track patient outcomes, and support quality improvement initiatives.
Job Description
Minimum Qualifications :
1.
High school diploma or equivalent.
Licensed Practical Nurse (LPN).
Two (2) years of relevant nursing experience.
Preferred Qualifications :
1.
Three (3) years of relevant nursing experience.

Physical Requirements :
Prolonged, extensive, or considerable standing / walking
Lifts, positions, pushes and / or transfer patients and equipment
Considerable reaching, stooping, bending, kneeling, crouching
Frequent exposure to hazardous chemicals, sick patients, bodily substances, noise and
possible exposure
to radiation, lasers, electric shock, etc.
Regularly exposed to the risk of bloodborne diseases and other transmissible infections
Contact with patients under
wide
variety of circumstances
Subject to varying and unpredictable situations
Handle emergency and crisis situations
Subjected to irregular hours
May have contact with hazardous materials
Skills & Abilities : 1.
Ability to effectively communicate with patients, families, physicians, and healthcare team.

Knowledge and application of the nursing process to implement a
nursing
plan of care.
Possesses and applies the skills and knowledge necessary to provide care to patients throughout the life span, with consideration of aging processes, human development stages and cultural patterns in each step of the care process.
Full time, 40 hours a week. The structure of the week may be negotiated. 4-10- hour shift vs. 5-8- hour shift
